Basic Membership

$1,750/yr

Designed for individuals with a single endocrine condition (excluding diabetes) that needs more than one visit/yr. No referral required. Includes ONE of any of these diagnoses 

  • Thyroid disease [Hypothyroidism, Hashimotos, Graves, Hyperthyroid, Thyroid Nodules, Thyroid Cancer, Thyroid Disease in pregnancy]

  • Osteopenia/Osteoporosis

Follow up Visit ($300)

Ideal for non-diabetic patients with stable endocrine conditions requiring focused care through a single annual visit.

  • This streamlined option includes diagnostic test ordering and completion in advance of your visit so that we can review your test results at the time of these scheduled appointments and adjust treatments as needed.

  • This approach does NOT include inbetween-visit communication or prescriptions and will require another visit to address additional needs

  • For patients requiring more frequent care, we offer an easy transition to a membership program, which is perfect for those who need expert intermittent*** more frequent assessment and management of their endocrine condition while continuing routine medical care with their primary provider

New Patient Consultation

This visit is required for new patients to establish care with Dr. Joseph-Hayes after which a plan for future follow-up will be determined

Initial Consult ($450)

Start with a comprehensive 60-minute consultation

This will include

  • a detailed review of your medical history, outside records, imaging, current concerns and symptoms and a physical exam.

  • CGM and insulin pump downloads will be completed during the visit if indicated

  • Any needed lab and imaging tests will be ordered at this visit.

  • a 15-minute follow-up phone call within 30 days to discuss results and recommendations and determine next steps

For those patients who have already been established with Dr. Joseph-Hayes at her previous practice, A new patient consultation is not needed as long as you have been seen within the last 3 years. please call to determine the best plan option for you
